JOB DESCRIPTION - Community Support Facilitator
- (Paid Roommate)
The Community Support Facilitator (CSF) will plan,
coordinate and provide individualized support to the individual
receiving Supported Living Services from TMI and will work under
the direct supervision of the Community Support Coordinator/Director
of Community Living and indirect supervision of the CEO and CFO.
The CSF will demonstrate effective job skills with persons with
disabilities, and a sensitivity to their needs and preferences.
The person who accepts this position must have the ability to work
with agency staff, family members, and private citizens. He/she
must be able to work independently and as a member of a team.
- This job is classified as an exempt, salaried, and a benefit
eligible position.
- The workday is based on the needs of the individual supported
by the agency. The supported
individual and the CSF will design a schedule with final approval
by the Coordinator which promotes optimum health and safety
for the supported individual.
- Weeknight and Weekend relief is available. Arranging relief
is the responsibility of the CSF. Weeknight relief hourly pay
is deducted from the regular salary of the CSF. Weekend relief
is paid exclusive of the regular salary and is optional income
for the CSF. The expectation is that the CSF will strive toward
a minimum of two full weekends off per month. The goal is to
reduce paid supports over time.
- Additional independent living support hours may be worked
provided you demonstrate good organizational
skills and continued quality support in your primary assignment
as a Paid Roommate. The number of hours and length of service
will be determined by your supervisor. Revocation of any additional
assignment(s) may be done at any time with reasonable notice.
