JOB DESCRIPTION - Companion/Relief

The Supported Living Companion will work temporary assignments under the direct supervision of the Community Support Coordinator/Director of Community Living and indirect supervision of the CEO and CFO. The companion must demonstrate effective skills with persons with disabilities, and a sensitivity to their needs and preferences. Must have the ability to work with agency staff, family members, and private citizens, and must be able to work independently and as a member of a team, while providing individualized supports at home and in their neighborhood.

Additional Qualifications:

• Comply with all Federal, State and/or local reporting and disclosure requirements.
• Provides individual assistive technology/adaptive equipment, or other support services.
• Collect data and completes any reports/paperwork requested by supervisor.
• Responsible for timely communication of all information to appropriate staff.
